Referrals need to be sent directly to us from your doctor’s office. Once you have downloaded our referral form (Download Form Here), fill out the patient portion and send the form to your doctor to complete and sign. Your doctor can also write his/her own referral.
Whether using our referral form or having a doctor write a referral, it should include:
Medical clearance should not be confused with clearance to drive independently.
Your past paperwork will help us better understand your condition, allowing us to tailor your evaluation and/or treatment to your specific needs. Having papers organized also saves us from searching for this information later, which could slow down the process.

This list is not intended to be all-inclusive, and not every item will apply in every case. If you do not have copies of these records, you may need to contact your healthcare provider(s). In some cases, you may be required to sign a release of information form before your provider will be willing to release your records to DRCE. Once your doctor has sent us your referral, we can schedule your first appointment one of two ways:
Your initial appointment will take 1 1/2 to 3 hours, depending on the complexity of your case.
You should arrive 10-15 minutes before your scheduled appointment time. You should not drive to this appointment. All evaluations, and the majority of trainings, are conducted in our specially-modified vehicles. If you are having trouble getting to your appointment, let us know. We may be able to help.
